Tendera is built for Arizona homes only. Its compliance categories, personnel-file items, service plan intervals, and incident form follow the Arizona assisted living rules (Arizona Administrative Code Title 9, Chapter 10, Article 8) rather than a generic national template.
What follows is a high-level map, not a rule summary. Each row links to the detailed guide, which carries the rule citations and the exceptions, and to the product workflow that supports it. Not legal advice.
Facility documents current, and drills on a schedule.
A surveyor asks for the facility's core documents on arrival. The emergency and safety rules set recurring drills: a disaster drill every three months on each shift and an evacuation drill every six months, each documented, with an elopement drill every six months per shift where directed care is authorized. Facility documents and drill log.
The facility license, fire inspection, and fall prevention program are tracked with expiration dates, and drills are logged in the app with date, type, participants, and outcome, or uploaded from paper. Missing and expiring items show on the Compliance hub.
A service plan for each resident, reviewed on the interval for their level of care.
Completed within 14 calendar days of acceptance, then reviewed and updated at least every 12 months for supervisory care, every 6 months for personal care, and every 3 months for directed care, and within 14 days of a significant change. R9-10-808
Versioned service plans with the next review date set by level of care.
Each plan carries the resident's level of care and its next review date on that interval. A scheduled review, a significant change, or a change in level opens a new version; participants and signatures are recorded and prior versions kept. Reviews coming due appear on the Compliance hub.
Medication activity that the resident's record can demonstrate.
The medication services rule ties orders, administration, assistance with self-administration, refusals, errors, and unexpected reactions back to the resident's medical record. Survey readiness depends on what the record shows, not on what a caregiver remembers. R9-10-817
Med Pass, or the paper MAR workflow.
Digital: each dose is prepared, then given, refused, or held with a structured reason, initials, and a signing PIN where the home requires one; hold parameters attach the vitals; a month-view MAR prints. On paper: monthly MAR packets are prepared per resident, marked printed by a staff member, reviewed by management, and followed up.
A personnel record for every employee, with the required evidence on file.
Qualifications, orientation, licenses or certifications, evidence of freedom from infectious tuberculosis on or before the start date, CPR and first aid documentation where required, and fingerprint clearance under state law. Employment application, reference check, TB test and screening, CPR and first aid, fingerprint clearance card, caregiver certificate and skills checklist, APS registry check, and memory care training, plus any document type you add. Expirations are visible before they lapse, and renewals keep the prior copy on file.
Incidents documented with the content the rule names.
For an accident, emergency, or injury that results in medical services: date and time, description, witnesses, caregiver actions, persons notified, and prevention action. Abuse, neglect, and exploitation allegations carry their own investigation record. Reporting pathways depend on the event and on program participation. R9-10-819(D) · R9-10-803(J)

What happened, vitals, injuries, medical response, contributing factors, prevention action, whether it is reportable, attachments, and a signature. Medications given in the prior 24 hours are attached automatically, notifications are recorded on a timeline, and the report prints. Deciding which pathway applies stays with the manager.
A resident record that is complete and available on request.
Move-in paperwork, health care directive status, the current service plan, medication records, and care documentation, producible when a surveyor or physician asks. What belongs in the record depends on the resident and on the facility's own policies.

One record per resident with an intake checklist that ticks itself when the matching document is uploaded. Face sheet, month-view MAR and ADL records, vitals, assessments, and incident reports each print in a click.
Readiness for an annual compliance inspection, at any time.
The Department may enter and inspect, and generally inspects each licensed facility at least annually. Documentation either exists on that day or it does not. A.R.S. § 36-424 · § 36-425
